Peripheral nerves and Schwann cells (SCs) are privileged and protected sites for initial colonization, survival, and spread of leprosy bacillus. Mycobacterium leprae strains that survive multidrug therapy show a metabolic inactivation that subsequently induces the recurrence of typical clinical manifestations of leprosy. Furthermore, the role of the cell wall phenolic glycolipid I (PGL-I) in the M. leprae internalization in SCs and the pathogenicity of M. leprae have been extensively known. This study assessed the infectivity in SCs of recurrent and non-recurrent M. leprae and their possible correlation with the genes involved in the PGL-I biosynthesis. The initial infectivity of non-recurrent strains in SCs was greater (27%) than a recurrent strain (6.5%). In addition, as the trials progressed, the infectivity of the recurrent and non-recurrent strains increased 2.5- and 2.0-fold, respectively; however, the maximum infectivity was displayed by non-recurrent strains at 12 days post-infection. On the other hand, qRT-PCR experiments showed that the transcription of key genes involved in PGL-I biosynthesis in non-recurrent strains was higher and faster (Day 3) than observed in the recurrent strain (Day 7). Thus, the results indicate that the capacity of PGL-I production is diminished in the recurrent strain, possibly affecting the infective capacity of these strains previously subjected to multidrug therapy. The present work opens the need to address more extensive and in-depth studies of the analysis of markers in the clinical isolates that indicate a possible future recurrence.

Objectives: Main therapy for leprosy reactions is 12 weeks corticosteroids according to World Health Organization recommendations, but recovery cannot be achieved and recurrence occurs. Long duration of administration was thought to provide better clinical improvement. Evidence of the efficacy of corticosteroids in leprosy reactions is still lacking, and optimal dose and duration of therapy vary, while the need for long-term high-dose corticosteroids makes it difficult to avoid adverse effects. Methods: This is a retrospective cohort study analyzing the difference between therapeutic effectiveness and adverse effects of 12 weeks and >12 weeks corticosteroids, involving all new leprosy patients without age restriction, at Cipto Mangunkusumo Hospital and Cakung Community Health Center in Indonesia during 1 January 2015-31 December 2017. Secondary data were collected from medical records, and observations carried out until December 2018. Therapeutic effectiveness was assessed from clinical improvement to corticosteroids discontinuation, without 3 months recurrence after first cycle was completed. Adverse effects were assessed by all corticosteroids-related side effects. Results: Of 195 patients, 57 (29.2%) used 12 weeks corticosteroids, and 138 (70.8%) for >12 weeks. Effectiveness occurred in 38 (66.7%) of 12 weeks group and 106 (76.8%) of >12 weeks group (relative risk = 0.604, 95% confidence interval = 0.307-1.189, p = 0.143). Of 145 patients, adverse effects occurred in 12 (31.6%) of 12 weeks group and 70 (65.4%) of >12 weeks group (relative risk = 0.244, 95% confidence interval = 0.111-0.538, p < 0.001). Of 171 adverse effects, 37.4% were mild such as dyspepsia, skin disorders, and lipodystrophy, while 62.6% were severe in the form of neuropsychiatric disorders, eye disorders, cardiovascular disease, gastrointestinal bleeding, metabolic-hormonal abnormalities, and reactivation of infections. Conclusion: There is no effectiveness difference in the form of clinical improvement without 3 months recurrence, between 12 weeks and >12 weeks corticosteroid, while longer administration causes 4 times more events.

Background Nail braces are reportedly effective for treating both acute inflamed and chronic dystrophic type ingrown toenails. Aims In this study, risk factors for poorly controlled and recurrence-prone ingrown toenails treated with nail braces were identified. Methods We performed a retrospective study on patients with ingrown toenails between June 1, 2015, and May 31, 2018. The last follow-up date was January 31, 2019. Multivariate logistic regression was performed to evaluate the possible factors associated with poorly controlled status (ongoing paronychia during treatment) and recurrence. Results There were 120 (244 sides) and 118 patients (167 sides) with chronic dystrophic and acute inflamed type ingrown toenails, respectively. The mean treatment duration and follow-up period were 161.2 ± 98.3 days and 432.7 ± 320.9 days, respectively. Poor control and recurrence were seen in 7.3% (17/232) and 12.2% (27/221) of the patients, respectively. In the multivariate analysis, acute inflamed ingrown toenails, previous nail avulsion, proximal nail fold hypertrophy and more than one affected side remained significantly associated with poorly controlled ingrown toenails. Foot bone deformity was significantly associated with recurrence. Limitations This study was a retrospective study so that confounding factors such as comorbidities, body mass index, accompanying nail changes and lifestyle could not be evaluated. Conclusion Several risk factors related to poor control and recurrence were identified. Patients could therefore benefit from more suitable treatment plans with reasonable expectation.

The 10-year results after national introduction of pelvic lymph node staging in Danish intermediate-risk endometrial cancer patients not given postoperative radiotherapy. OBJECTIVE: To prepare for the national introduction of sentinel node staging, we evaluated the consequences of the previous national decision to introduce lymph node staging in intermediate-risk endometrial cancer patients (grade 1/2 with > 50% or grade 3 with < 50% myometrial invasion) by determining the number of patients upstaged by lymphadenectomy and whether upstaging affected the survival and recurrence patterns of non-staged patients and patients with and without lymph node metastases. STUDY DESIGN: In a national cohort study, 2005-12, 1294 stage I-IV patients who should have been offered lymphadenectomy were progressively registered. The number of patients upstaged by lymphadenectomy, 10-year survivals were evaluated by Kaplan-Meier analysis and adjusted Cox regression. RESULTS: This study demonstrates that it takes time to introduce lymphadenectomy at a national level, as indicated by the increasing number of cases staged per year, from 12% in 2005 to 74% in 2012. Pelvic lymphadenectomy was performed in 43.8% (567/1294) and lymph node metastases were found in 13.6% (77/567). As 54 patients had further dissemination outside the uterine body, only 23 patients (6%) were upstaged from stage I to IIIC. Compared to lymph node-negative patients, the 77 patients with lymph node metastasis had significantly lower overall, (55% versus 68%), disease-specific (64% versus 86%), and progression-free survival (51% versus 77%), mainly due to non-local recurrences including a high number of paraaortic recurrences. In 873 final stage I intermediate-risk patients, 10-year survival and recurrence rates were not significantly lower in non-staged as compared to lymph node-negative patients (overall survival 62% versus 70%: disease-specific survival: 90% versus 90%, progression-free survival: 81% vs 83%), probably due to the low number of patients upstaged from stage I to stage IIIC. CONCLUSION: Lymph node metastases were present in 13.6% of patients with intermediate-risk who underwent pelvic lympadenectomy, and these patients had a lower 10-year survival than lymph node-negative patients. Because lymphadenectomy upstaged only 6% from stage I to stage IIIC, survival and recurrence rates were not significantly compromised in non-staged as compared to lymph node-negative intermediate-risk stage I patients. Sentinel node staging has now been implemented in Danish intermediate-risk endometrial cancer patients.

BACKGROUND: Mycosis fungoides is the most common form of cutaneous T-cell lymphoma. Narrowband ultraviolet B and psoralen and ultraviolet A are effective treatment options, but studies of their treatment efficacy and disease relapse remain limited. OBJECTIVES: This study aimed (1) to determine the efficacy of narrowband ultraviolet B and psoralen and ultraviolet A as a treatment for early-stage mycosis fungoides and explore the predictive factors for complete remission and (2) to determine the relapse rate and analyze their predictive factors, including the utility of maintenance therapy. METHODS: This was a retrospective cohort study consisting of 61 patients with early-stage mycosis fungoides (IA - IB) treated with narrowband ultraviolet B or psoralen and ultraviolet A as the first-line therapy from January 2002 to December 2018 at the Division of Dermatology, Ramathibodi Hospital, Bangkok, Thailand. Cox regression analysis and Kaplan-Meier survival curve were performed for the main outcomes. RESULTS: A complete remission was achieved by 57 (93.5%) patients. The median time to remission was 7.80 ± 0.27 months. Types of phototherapy (narrowband ultraviolet B or psoralen and ultraviolet A), age and gender did not associate with time to remission, while the presence of poikiloderma and higher disease stage led to a longer time to remission. The cumulative incidence of relapse was 50.8%. The median time to relapse was 24.78 ± 5.48 months. In patients receiving phototherapy during the maintenance period, a treatment duration longer than six months was associated with a significantly longer relapse-free interval. CONCLUSION: Narrow-band-ultraviolet B and psoralen and ultraviolet A are effective treatment options for early-stage mycosis fungoides. Maintenance treatment by phototherapy for at least six months seems to prolong remission.

Abstract INTRODUCTION: This study analyzed the magnitude and temporal trends of leprosy relapse in Ceará in 2001-2018. METHODS: Descriptive cross-sectional and ecological-time trend studies were performed. RESULTS: We diagnosed 1,777 leprosy relapse cases. Higher prevalence of relapse was observed in men, illiterates, mixed race, multibacillary leprosy, lepromatous leprosy, and persons with visible disabilities. The proportion of relapse increased throughout the study period. CONCLUSIONS: Leprosy relapse is prevalent in certain groups.

Abstract INTRODUCTION: Leprosy recurrence is the reappearance of the disease after treatment with current schemes and discharged for cure and may have variable incubation periods. METHODS: This is a descriptive observational study of leprosy recurrence in Espírito Santo diagnosed between January 2018 and January 2020. RESULTS: One hundred and ninety-two cases were available, of which 30 were diagnosed with leprosy recurrence. CONCLUSIONS: In 25 cases, the incubation period was 5-15 years after the first treatment, favoring bacillary persistence. In the remaining 5 cases, the disease had recurred after 15 years, pointing to reinfection as none of them exhibited drug resistance.

BACKGROUND: Penoscrotal extramammary Paget's disease is a rare, slow-growing neoplasm with high frequency of local recurrence. AIMS: The aim of this study was to investigate the difference in clinicopathological characteristics between first-time and recurrent penoscrotal Paget's disease, and to discover the potential risk factors of recurrence. METHODS: Between January 2007 and February 2014, a total of 164 Chinese patients with biopsy-proven tramammary Paget's diseaseex in penis and scrotum underwent wide local resection in our institution. Among them, 142 patients with first-time disease and other 22 patients with recurrent disease were enrolled in this retrospective analysis. RESULTS: The median duration of symptoms was much shorter in recurrent disease than in first-timers (3 vs. 24 months, P < 0.001). Patients with recurrent disease tended to have lower lesion exudation rates (27.3% vs. 51.8%, P= 0.032). In addition, patients with distant stage were more likely to obtain recurrent disease compared with first-time disease (P = 0.005). Through immunohistochemical detection of extramammary Paget's specimen, we found that HER2/neu protein expression in the recurrent group was significantly higher than first-timers (P = 0.036). LIMITATIONS: In this study, the information on familial history of most patients was insufficient. Moreover, due to the lack of follow-up data of our included cases, we were unable to evaluate the prognosis after diagnosis of extramammary Paget's disease. CONCLUSION: Patients with penoscrotal Paget's disease, especially those with shorter duration of symptoms, exudation of lesions, distant-stage, Paget cells infiltrating into adnexa, and HER2/neu expression, should be followed up more carefully after surgery, as they were more likely to suffer recurrence.

Introdução: Lacaziose é uma doença rara que afeta principalmente trabalhadores de áreas tropicais, sendo descritos aproximadamente 500 casos no mundo. A lacaziose é um doença parasitária causada pelo fungo saprófita Lacazia loboi, para o qual não existe um tratamento específico. A cirurgia é o tratamento mais eficiente para as deformidades causadas pela doença. Entretanto, é um tratamento temporário, uma vez que as recidivas são frequentes. Lacazia loboi acomete duas espécies de golfinhos, o Tursiops truncates e o Sotalia guianensis. A literatura aborda o tratamento cirúrgico de maneira superficial, pois não existem trabalhos específicos descrevendo o tratamento cirúrgico para essa doença. Métodos: Descrevemos aqui nossos 8 anos de experiência no Hospital de Base de Porto Velho-Rondônia com 22 casos submetidos a tratamento cirúrgico e acompanhados. Resultados: A maioria dos pacientes (91%) já se submeteram a pelo menos um tratamento cirúrgico associado ao tratamento antifúngico. Os pacientes apresentavam lesões com tempo de evolução entre 5 meses e 6 anos previamente ao tratamento cirúrgico. Apenas dois casos eram virgens de tratamento. Conclusão: Nossos pacientes foram acompanhados, mas apenas 11 dos 22 pacientes retornaram para acompanhamento. Recorrências foram observadas em 9 dos 11 pacientes, com um período de latência de 5 meses (AU)


Introduction: Lacaziosis is a rare disease that mainly affects workers in tropical areas, with approximately 500 cases reported worldwide. Lacaziosis is a parasitic disease caused by the saprophytic fungus Lacazia loboi; there is no specific treatment for this disease. Surgery is the most effective treatment for the deformities caused by the disease. However, it is a temporary treatment, since disease recurrence is frequently observed. Lacazia loboi affects two species of dolphin, Tursiops truncates and Sotalia guianensis. The available literature discusses the surgical treatment in a superficial way , because there are no specific studies describing the surgical treatment for this disease. Methods: Here, we describe our 8 years of experience with lacaziosis at the Hospital de Base de Porto Velho - Rondônia; a total of 22 patients underwent surgical treatment and were followed-up. Results: The majority of the patients (91%) had already submitted to at least one surgical treatment together with antifungal treatment. The patients presented with lesions with disease progression ranging from 5 months to 6 years prior to surgical treatment. Only two patients were treatment-naive. Conclusion: Our patients were followed-up; however, only 11 of the 22 patients returned for follow-up. Recurrences were observed in 9 of the 11 patients, with a latency period of 5 months.(AU)

OBJETIVO: descrever as características clínicas e epidemiológicas dos casos de recidivas de hanseníase diagnosticados nos serviços de referência do município de Teresina, Piauí. MÉTODOS: estudo de série de casos realizado na Clínica de Dermatologia do Hospital Getúlio Vargas e no Centro Maria Imaculada, no período de janeiro de 2001 a dezembro de 2008, com dados do Sistema de Informação de Agravos de Notificação (Sinan) conferidos nos prontuários. RESULTADOS: foram confirmados 76 casos de recidivas nos serviços de referência, com média de idade de 44,2 anos; o tempo médio da alta em relação ao aparecimento da recidiva foi de 7,9 anos; os sinais e sintomas que caracterizaram as recidivas nas duas unidades estudadas foram manchas, infiltração, nódulos e placas; a conduta introduzida foi a multidrogaterapia. CONCLUSÃO: os casos de recidivas diagnosticados nos serviços de referência de Teresina possuem características clínicas semelhantes e apresentam perfil epidemiológico condizente com o do Brasil.


OBJECTIVE: To describe clinical and epidemiological characteristics of leprosy recurrence diagnosed at referemce centers in the city of Teresina, Piaui, Brazil. METHODS: Case series study conducted at the Getulio Vargas Hospital Dermatology Clinic and at the Maria Imaculada Center, from January 2001 to December 2008, using Notifiable Diseases Information System data checked against patients' records. RESULTS: 76 cases of leprosy recurrence cases were confirmed with average patient age of 44.2 years. The average time between discharge and recurrence was 7.9 years. The signs and symptoms of recurrence in both clinics were skin patches, infiltration, nodules and plaques. Multidrug therapy was used. CONCLUSIONS: Recurring cases diagnosed at the clinics in Teresina have clinical characteristics and an epidemiological profile similar to those of Brazil as a whole.

CONCLUSIÓN: los casos de recurrencia diagnosticados en los servicios de referencia de Teresina tienen características clínicas similares y el perfil epidemiológico es consistente con el de Brasil.

RESUMO

Relapsed cases of leprosy are not common in patients treated regularly with the recommended polychemotherapeutical schemes. Relapses must be readily identified and distinguished from reactions. This article reports a clinical case of relapse in leprosy, with atypical and exuberant skin changes in patient treated for eleven years with a paucibacillary scheme.

This article presents a case of relapse, with isolated neural manifestation, in a multibacillary patient previously treated with multidrug therapy for multibacillary leprosy (24 doses). The patient returned to the service six years after the end of treatment, with pain in hands and legs. He was investigated, and the serological monitoring showed an important increase in anti-phenolic glycolipid serum levels. A neural recurrence was suspected, since the patient had no new skin lesions. A new biopsy in the right ulnar nerve showed a bacilloscopy of 2 +, compatible with relapse. This is a literature review of the etiological, clinical, propedeutical and diagnostic aspects of this situation so poorly understood.


O presente artigo relata um caso de recidiva, com manifestação neural isolada, em paciente multibacilar previamente tratado com poliquimioterapia para multibacilar 24 doses. O paciente retorna ao serviço, seis anos depois do fim do tratamento, com dores em mãos e pernas. Na investigação, o acompanhamento da sorologia anti-glicolipídeo fenólico 1 demonstrou aumento importante dos níveis séricos, e foi aventada a hipótese de recidiva neural, já que o paciente não apresentava lesões cutâneas novas. Uma nova biópsia, em nervo ulnar direito, demonstrou baciloscopia de 2+, compatível com recidiva. Faz-se revisão da literatura sobre aspectos etiológicos, clínicos, propedêuticos e diagnósticos dessa situação tão pouco compreendida.

OBJETIVO: Analisar fatores associados à ocorrência de recidiva em hanseníase. MÉTODOS: Estudo retrospectivo caso-controle com 159 pacientes maiores de 15 anos diagnosticados com hanseníase em cinco municípios do estado de Mato Grosso, cujas unidades de saúde eram consideradas de referência para o atendimento. O grupo de casos incluiu 53 indivíduos com recidiva de 2005 a 2007 e foi comparado ao grupo controle (106 com alta por cura em 2005), pareados por sexo e classificação operacional. Foram usados dados do Sistema de Informação de Agravos de Notificação, prontuários e entrevistas. Utilizou-se regressão logística condicional e abordagem hierárquica. RESULTADOS: Após análise ajustada, mostraram-se associados à ocorrência de recidiva: indivíduos residentes em casas alugadas (OR = 4,1; IC95 por cento:1,43;12,04), em domicílio de madeira/taipa (OR = 3,2; IC95 por cento: 1,16;8,76), que moravam com mais de cinco pessoas (OR = 2,1; IC95 por cento : 1,03;4,36), com transtorno de uso de álcool (OR = 2,8; IC95 por cento: 1,17;6,79), irregularidade do tratamento (OR = 3,8;IC95 por cento: 1,44;10,02), sem esclarecimento sobre a doença/tratamento (OR = 2,6; IC95 por cento: 1,09;6,13), que usavam transporte coletivo para o acesso à unidade de saúde (OR = 5,5; IC95 por cento: 2,36;12,63), forma clínica da doença (OR = 7,1; IC95 por cento: 2,48;20,52) e esquema terapêutico (OR = 3,7; IC95 por cento: 1,49;9,11). CONCLUSÕES: Os fatores preditivos de recidiva relacionam-se com condições de moradia, hábitos de vida, organização dos serviços de saúde, formas clínicas e esquemas terapêuticos. Cabe aos serviços de saúde oferecer orientações adequadas aos pacientes, bem como garantir a regularidade do tratamento.


OBJECTIVE: To analyze factors associated with relapse of leprosy. METHODS: Retrospective case-control study including 159 patients older than 15 diagnosed with leprosy attending reference centers for leprosy in five municipalities in the state of Mato Grosso, central-western Brazil. Cases (n=53) were patients with relapsed leprosy diagnosed from 2005 to 2007 who were compared with controls (n=106) matching for gender and operational classification who were considered cured after treatment in 2005. Data was obtained from the local Notifiable Diseases Database, medical records and interviews. For the analyses conditional logistic regression and hierarchical approaches were used. RESULTS: After adjustment, the following factors were associated with relapse of leprosy: living in rental housing (OR = 4.1; 95 percentCI: 1.43;12.04); living in houses constructed of wood and mud (OR = 3.2; 95 percentCI: 1.16;8.76); living with dwellings with more than five people (OR = 2.1; 95 percentCI: 1.03;4.36); alcohol use disorder (OR = 2.8; 95 percentCI: 1.17;6.79); irregular treatment (OR =3.8; 95 percentCI: 1.44;10.02); lack of knowledge about the disease/treatment (OR = 2.6; 95 percentCI: 1.09;6.13); use of public transportation to get to the clinic (OR = 5.5; 95 percentCI: 2.36;12.63); clinical form of the disease (OR = 7.1; 95 percentCI: 2.48;20.52), and treatment regimen (OR = 3.7; 95 percentCI: 1.49;9.11). CONCLUSIONS: The predictive factors of relapse are associated with housing conditions, living habits, organization of health services, clinical forms of leprosy and treatment regimen. Health services should educate patients on the disease as well as ensure consistent treatment.


OBJETIVO: Analizar factores asociados a la ocurrencia de recidiva en hanseníasis. MÉTODOS: Estudio retrospectivo caso-control con 159 pacientes mayores de 15 años diagnosticados con hanseníasis en cinco municipios del Estado de Mato Grosso, Centro-oeste de Brasil, cuyas unidades de salud eran consideradas de referencia para el atendimiento. El grupo de casos incluyó 53 individuos con recidiva de 2005 a 2007 y fue comparado con el grupo control (106 con alta por cura en 2005), pareados por sexo y clasificación operacional. Se usaron datos del Sistema de Información de Agravios de Notificación, Prontuarios y entrevistas. Se utilizó regresión logística condicional y abordaje jerárquico. RESULTADOS: Posterior al análisis ajustado, se mostraron asociados a la ocurrencia de recidiva: individuos residentes en casas alquiladas (OR=4,1; IC95 por ciento:1,43;12,04), en domicilio de madera/tapia (OR=3,2; IC 95 por ciento:1,16;8,76), que moraban con más de cinco personas (OR=2,1; IC95 por ciento:1,03;4,36), con trastorno por uso de alcohol (OR=2,8;IC95 por ciento:1,17;6,79), irregularidad del tratamiento (OR= 3,8; IC95 por ciento: 1,44;10,02), sin esclarecimiento sobre la enfermedad/tratamiento (OR= 2,6; IC95 por ciento:1,09,6,13), que usaban transporte colectivo para el acceso a la unidad de salud (OR=5,5; IC95 por ciento: 2,36;12,63), forma clínica de la enfermedad (OR= 7,1;IC95 por ciento: 2,48;20,52) y esquema terapéutico (OR= 3,7; IC95 por ciento:1,49;9,11). CONCLUSIONES: Los factores predictivos de recidiva se relacionan con condiciones de vivienda, hábitos de vida, organización de los servicios de salud, formas clínicas y esquemas terapéuticos. Compete a los servicios de salud ofrecer orientaciones adecuadas a los pacientes, así como garantizar la regularidad del tratamiento.

INTRODUÇÃO: Em 1997, após a realização de estudo multicêntrico, duplo cego e randomizado, em nove centros de tratamento de hanseníase na Índia, o Ministério da Saúde adotou o esquema alternativo dose única ROM para casos de lesão única, paucibacilar, sem nervo periférico afetado, índice baciloscópico negativo, em Centros de Referência da doença no Brasil. O estudo se propôs a avaliar a efetividade do esquema ROM em pacientes tratados no período de 1997 a 1999 no Serviço de Dermatologia da Santa Casa de Vitória. MÉTODOS: Foram selecionados e tratados com o esquema ROM, 54 pacientes das formas indeterminada e tuberculóide. Estes pacientes foram convocados de março a outubro de 2006 para reavaliação clínica. RESULTADOS: Vinte e nove pacientes avaliados (85,2 por cento; IC95 por cento: 70-100,4) estavam curados, cinco (14,7 por cento; IC95 por cento: 7,4-22,0) recidivaram e 20 pacientes não retornaram; porém, não havia outra notificação de reingresso ao tratamento no banco de dados da Secretaria Estadual de Saúde. CONCLUSÕES: O estudo evidenciou taxa de cura de 90,8 por cento e taxa bruta de recidiva de 9,2 por cento, após período de sete a nove anos da dose ROM. O tratamento alternativo ROM demonstrou melhor efetividade para lesão única menor que quatro centímetros e aparecimento há menos de cinco anos.


INTRODUCTION: In 1997, after obtaining a combined multi-state double-blind randomly controlled clinical trial study from nine Indian centers involved in the treatment of Hansen's Disease, the Ministry of Health adapted the single dose ROM Therapy approach in those cases involving the treatment of a single skin lesion, paucibacillary leprosy without evidence of peripheral nerve trunk involvement and indication of negative baciloscope, in the Referral Centers in Brazil. The study aimed to evaluate the effectiveness of the single dose ROM Therapy approach in those patients who were treated from the period of 1997 to 1999 in the Ambulatory Dermatologic Unit in the Hospital in Vitória, ES. METHODS: Fifty-four patients with tuberculoid and indeterminate leprosy were selected and treated with the single dose ROM Therapy approach. These patients were contacted from March 2006 up and until October 2006 for further clinical reevaluation. RESULTS: From the studies conducted, the following results were found to exist: 29 patients (85,2 percent; 95 percentCI: 70-100,4) were cured, 5 patients (14,7 percent; 95 percentCI: 7,4-22,0) relapsed, and 20 patients didn't return; however, there are no additional records of any notification of other treatment(s) in the State Department of Health's informational data banks. CONCLUSIONS: The study demonstrated a rate of cure of 90.8 percent and a rate of relapse of 9.2 percent after a period of seven to nine years using the single dose ROM Therapy approach. Additionally, this alternative treatment further demonstrated a better effectiveness for a single skin lesion smaller than four centimeters and with an appearance in less than five years.

Recidiva de hanseníase é o reaparecimento da doença, após tratamento regular com os esquemas vigentes e alta por cura. Neste estudo de coorte retrospectivo e descritivo o objetivo foi avaliar as características dos casos de recidiva de hanseníase no Estado do Espírito Santo entre 2000 e 2005. As estratégias de investigação foram: monitoramento das entradas no SINAN, análise das fichas e discussão dos casos no Centro de Referência Estadual. Foram estudados 104 casos de recidiva, representando 1,12 por cento em relação aos casos novos diagnosticados no período. A maior freqüência foi entre 21 a 60 anos; 59,6 por cento eram do sexo masculino; 44,2 por cento apresentaram a recidiva após cinco anos da alta; 66,4 por cento eram multibacilares, sendo 42,2 por cento com baciloscopias positivas (bacilos íntegros), portanto recidivas. Baciloscopias negativas foram observadas em 57,8 por cento. Estudos prospectivos devem ser feitos para estabelecimento da taxa real de recidiva.


Leprosy relapse is the reappearance of the disease after regular treatment with current regimens and discharge due to cure. In this retrospective and descriptive cohort study, the aim was to evaluate the characteristics of cases of leprosy relapse in the State of Espírito Santo between 2000 and 2005. The investigation strategies consisted of monitoring the SINAN entries, with file analysis and case discussions at the State Reference Center. One hundred and four cases of relapse were studied, representing 1.12 percent of the new cases detected over this period. The greatest frequency was between 21 and 60 years of age; 59.6 percent were men; 44.2 percent presented relapse more than five years after discharge; 66.4 percent were multibacillary; and 42.2 percent presented positive bacilloscopy (complete bacilli) and therefore were relapse cases. Negatives bacilloscopy was observed in 57.8 percent. Prospective studies should be conducted to establish the real relapse rate.
